Social Worker – Full-Time | Days | Fort Walton Beach, FL

• Shift: Days with rotating weekends (8:00 AM – 4:30 PM or 9:00 AM – 6:00 PM)
• Pay Range: $24.93 – $33.59/hr (based on experience)
• Sign-On Bonus: $5,000 (paid in 5 installments over 2 years)
• Relocation Assistance: Available, case by case

We are hiring a Master’s-level Licensed Clinical Social Worker (LCSW) to join our hospital-based interdisciplinary team. This role focuses on non-clinical psychosocial assessments, discharge planning, resource navigation, and crisis interventions for patients in both inpatient units and the emergency department.

Responsibilities:

• Conduct biopsychosocial assessments and coordinate discharge plans for high-risk patients

• Support patients and families with emotional, social, and financial challenges

• Provide crisis intervention, including end-of-life, terminal illness, and traumatic events

• Collaborate with case managers, physicians, and interdisciplinary staff

• Educate patients and families on available community and healthcare resources

• Maintain strong working knowledge of Medicare/Medicaid/private insurance rules

Qualifications:

• Master’s Degree in Social Work (MSW) required

• LCSW (Licensed Clinical Social Worker) required

• Minimum 2 years of social work experience (hospital, county, or payer experience preferred)

• Highly organized, self-starter with strong critical thinking and multitasking skills

• Must be comfortable managing complex patient cases independently in a fast-paced environment

Preferred Backgrounds:

• Experience in a hospital, insurance company, or county-based social services

• Strong communication, documentation, and time management skills
